Maggots Debridement Therapy

Maggot Therapy for Treating Diabetic Foot Ulcers Unresponsive to Conventional Therapy

Maggot Debridement Therapy (MDT) is the medical use of live maggots (fly larvae) for cleaning non-healing wounds. Medicinal maggots have three actions: 1) they debride (clean) wounds by dissolving the dead (necrotic), infected tissue; 2) they disinfect the wound, by killing bacteria; and 3) they stimulate wound healing.

Maggot debridement therapy Larvae used for MDT need to be sterile to prevent contamination, and therefore must be bred in a controlled sterile, moist environment. Newborn larvae should be used within 8 hours or stored in a refrigerator at 8º to 10ºC, so as to slow their metabolism. To maximise debridement, it is important to ensure an optimal body temperature, adequate oxygen supply and moisture, though too much moisture may kill the larvae. The use of occlusive dressings should be avoided, as larvae require oxygen to survive. Propylene glycol from hydrogel dressings can limit the growth and viability of larvae, while systemic antibiotics do not affect larval development.

Mechanisms of maggot debridement Therapy

Debridement

Scientists first postulated that the debriding action of maggots was due to their mechanical wriggling. Maggots use a pair of mandibles/hooks for movement and attachment, and it was believed that the probing from the hooks may facilitate wound debridement. Recently, three proteolytic enzyme classes have been identified in the maggot excretions/ secretions (ES). These enzymes effectively degrade extracellular matrix components, including laminin and fibronectin. The ES could thus assist in the digestion of the wound matrix, leading to effective debridement.

Disinfection

The presence of antibacterial substances has long been identified in maggot ES. Thus, ES has an inhibitory effect on Gram-positive and Gram-negative bacteria, including methicillin-sensitive Staphylococcus aureus, methicillin-resistant S aureus (MRSA), Escherichia coli and Pseudomonas aeruginosa. This activity of the ES was thermally stable and protease-resistant. Using ultrafiltration, the latter study identified two fractions with inhibitory effects on S aureus and MRSA. It was also believed that ammonia excreted by maggots increases wound pH, thereby creating an unfavourable environment for bacterial growth. According to another hypothesis, maggots ingest bacteria and kill them within their alimentary tract. Mumcuoglu et al investigated the fate of E coli after ingestion by L sericata. It transpired that the proximal alimentary canal was more heavily infested, with 67% of the throat harbouring live bacteria. The figures dropped dramatically as the bacteria moved distally along their alimentary canal. Only 18% of the posterior hindgut was found to contain live bacteria.

Enhancement of wound healing

It was believed that the enhancement in tissue growth was due to an increase in fibroblast proliferation brought about by the ES. Horobin et al demonstrated that the ES altered fibroblast adhesions to collagen and fibronectin, and it was subsequently shown that it increased the migration (but not proliferation) of fibroblasts. This was attributed mainly to the action of serine and metallo proteinases. These investigators then developed a three-dimensional model to better simulate a human wound.Their results were consistent with the previous studies and supported by later investigators. An upregulation oftyrosine phosphorylation was also detected, which possibly enhanced the motility of the fibroblasts. Others have postulated that maggots secreted cytokines, which help wound healing. High levels of gamma-interferon and interleukin-10 (IL-10) were found in the ES, but as to whether these cytokines are responsible for increasing granulation requires further investigation.

Indications for maggot debridement therapy

Maggot debridement therapy is mainly used for the cleaning and disinfection of chronic wounds that are sloughy, necrotic, and infected. Various clinical studies have demonstrated the efficacy of MDT in treating wounds that fail to heal following alternative forms of treatment. Larvae were effective in removing necrotic tissue and exudation without damaging adjacent healthy tissue. This action stimulated tissue granulation and reduced offensive odours brought about by infections. Compared to conventional hydrogel therapy, MDT was more effective for chronic venous ulcers, diabetic ulcers, and pressure ulcers

Contra-indications and side-effects

Dry wounds are a relative contra-indication as maggots require a moist environment. The use of maggots should also be avoided in open wounds of body cavities or wounds in close proximity to large blood vessels so as to facilitate the removal of the larvae. Also, maggots should not be used in patients who are allergic to eggs, soybeans, or fly larvae. Maggot debridement therapy has not been associated with major adverse effects or complications, but has been reported to cause mild discomfort, malodour at the first change of dressing, and escape of larvae. Excessive pressure applied on to parts of the wound may also kill maggots in that area, leading to uneven wound debridement. Despite

these shortcomings, even patients experiencing pain during treatment tend to report improved appearance.

Advantages and disadvantages of maggot debridement therapy (“MDT")
  • Efficacy, as demonstrated in several small but significant controlled clinical studies.
  • Takes about 15-30 minutes to apply a secure dressing to keep the maggots in place.
  • Maggots are highly perishable and should be used within 24 hours of arrival.
  • Excellent safety record.
  • Simple enough that non-surgeons can use it to provide thorough debridement when surgery is not available or is not the optimal choice. This means that it is also possible to provide surgical quality debridement as an outpatient or in the home.
  • Low cost of treatment.

What is cupping (hijama)?

Cupping (hijama) is the best remedy recommended and used by the Messenger (Sallallaahu alayhi Wasallam). The Messenger (Sallallaahu alayhi Wasallam) said, "Indeed the best of remedies you have is cupping (hijama) ..." [Saheeh al-Bukhaaree (5371)]. He (Sallallaahu alayhi Wasallam) also said that on the night of Israa (his ascension to the heavens) he (Sallallaahu alayhi Wasallam) did not pass by an angel except that it said to him, "Oh Muhammad, order your Ummah (nation) with cupping (hijama)." [Saheeh Sunan Tirmidhee (3479)]. This shows the importance and greatness of this Sunnah.

'Hijama' in arabic is derived from 'hajm' which means 'sucking'. Cupping (hijama) is the process of applying cups to various points on the body by removing the air inside the cups to form a vacuum.

Cupping (hijama) is of three types:

* dry cupping (hijama) - This is the process of using a vacuum on different areas of the body in order to gather the blood in that area without incisions (small, light scratches using a sterile surgical blade or disinfected razor).

* Dry massage cupping (hijama). This is similar to dry cupping (hijama) but olive oil is applied to the skin (before applying the cups) in order to allow easy movement of the cups.

70% of diseases, pains and ailments are due to the blood being unable to reach certain parts of the body. Dry cupping (hijama) and dry massaging cupping (hijama) allow the blood to reach these places.

Dry and massage cupping (hijama) may be self-administered in the comfort and privacy of your own home. Each cupping (hijama) box is accompanied by an instruction booklet. We have also provided further instructions on our 'How To Cup' page.

Dry and massage cupping (hijama) may be administered any time of the day, any day of the week or month. There are no restrictions.

Although dry and massage cupping (hijama) are very beneficial, they are not from the Sunnah. However, they fall under the general hadeeth:

The Messenger (Sallallaahu alayhi Wasallam) said, "For every disease there is a cure so if the medicine comes upon the disease it cures it by the will of Allaah, the Most High." [Saheeh Muslim (2204)].

* wet cupping (hijama) - This is the process of using a vacuum at different points on the body but with incisions in order to remove 'harmful' blood which lies just beneath the surface of the skin. (It is recommended that wet cupping (hijama) is only administered by a cupping therapist).

Wet cupping (hijama) is best administered at certain times of the day and days of the week. Please see our 'Authentic Narrations' page for further details. Wet cupping (hijama) is from the Sunnah. To obtain maximum benefit from cupping (hijama), it is recommended that all three methods of cupping are used.

Is cupping (hijama) a cure for every disease?

Cupping (hijama) is a cure for every disease if performed in its correct time. The Messenger (Sallallaahu alayhi Wasallam) said, "Indeed in cupping (hijama) there is a cure." [Saheeh Muslim (5706)]. The Messenger (Sallallaahu alayhi Wasallam) said, "Whoever performs cupping (hijama) on the 17th, 19th or 21st day (of the Islamic, lunar month) then it is a cure for every disease." [Saheeh Sunan abi Dawud (3861)].

There are specific points on the body where the cups are applied for each ailment. Please see our 'Where To Cup' page for further details.

Is cupping (hijama) from the Sunnah?

Above are just some of the authentic narrations which show that cupping (hijama) is from the Sunnah of the Messenger (Sallallaahu alayhi Wasallam).

The Messenger (Sallallaahu alayhi Wasallam) said, "Whoever revives a Sunnah from my Sunnah and the people practise it, s/he will have the same reward of those who practise it without their reward diminishing" [Sunan ibn Maajah (209)].

Diabetes causes your blood sugar levels to be higher than normal. Over time, high blood sugar levels can damage the blood vessels and nerves in the body. Damage to the nerves means that you may have burning pain or lose feeling in a part of your body (this is called diabetic neuropathy). Damage to the blood vessels means that your feet may not be getting a good supply of blood.

How to avoid problems with your feet

Keep your blood sugar levels as close to normal as possible. Also, follow your doctor’s advice on diet, exercise and medicine. Here are some other ways to protect your feet:

Examine your feet everyday. Examine from the top to the bottom of your foot. Look and feel in between your toes and around the heel area. Look for cuts, sores, blisters, redness, swellings, corns and calluses, ingrown toe nails, changes in skin colour and so on. If you are unable to see your feet, use a mirror or ask someone to help you.

Wash your feet everyday with lukewarm (not hot) water and mild soap.

Do not soak your feet as this will make your skin too soft and fragile.

Do not use rough massage gloves or brushes as they can damage your skin.

Do not wash your feet for too long.

Dry your feet thoroughly with a soft towel and pat gently; don’t rub. Make sure you do not forget to dry between your toes. Damp skin can easily lead to infections.

Dust your feet with talcum powder, which will help keep them dry.

If you have dry skin on your feet, use a moisturising lotion to prevent cracking. Do not apply between your toes, as this may lead to infection.

Keep your toenails trimmed. Trim them with toenail clippers after you have washed and dried your feet. Trim the nails following the shape of your toes, and not too short. Do not cut into the corners of the nail, which may trigger an ingrown toenail. File your toenails with nail file to smoothen them.

Rasps for removing callus, razor blades, pointed scissors and corn plasters are widely used instruments in pedicure. Unfortunately, none of these are suitable for taking care of the feet and nails of people with diabetes, since they can easily cause injuries or damage to the skin. Use a pumice stone to remove callus, especially after washing the feet. Gently rub the stone over the callus in one direction only. Continue to do this for five minutes every day till the callus is being removed.

Always wear sandals when visiting the swimming pool.

Do not go barefoot, even when indoors.

Don’t let your feet get too hot or too cold.

Do foot exercises and massage your feet everyday to stimulate the circulation.

DO NOT go to the foot massage parlour to massage your feet as it may injure them.

If your feet have any cuts or abrasions, clean the wound immediately with saline, apply an antiseptic cream and secure a sterile dressing to the area. Continue with dressing until the wound is healed. Seek medical treatment if the wound does not show any signs of healing after two days...

Foods for diabetics

HAVING diabetes does not mean that you have to stick to a rigid diet. In fact, a diabetic diet is almost the same as that for the healthy. Diabetes does not mean completely cutting out foods containing carbohydrates.

Proper nutrition management and healthy food choices are necessary to avoid complications. Here are 10 foods high in fibre, antioxidants, vitamins and minerals.

1 - Asparagus

IF you love asparagus, you’ll be glad to know that it’s a non-starchy vegetable with only five grams of carbohydrates per serving and nearly two grams of dietary fibre. It is also high in the B vitamin folate, vitamin C, and a health-promoting antioxidant called glutathione. Glutathione can help boost the immune system and promote lung health by protecting against viruses. Besides, the American Heart Association recommends including foods containing folate and other B vitamins in your diet to help lower homocysteine levels. A serving of asparagus is ½ cup, or 115g cooked, and provides 33 per cent of the daily recommendation of 400 micrograms of folate, according to the Food and Drug Administration.

2 - Yogurt

A SWEET treat that is creamy, delicious and good for you. An excellent source of calcium, which helps promote bone health and teeth as well as muscle and blood vessel function, yogurt is also a good source of energy-boosting vitamin B2 (riboflavin) and protein. It also provides zinc, which can be deficient in some people with diabetes and aids in immune function and wound healing. Probiotic yogurt contains health-promoting bacteria that is beneficial for digestive health, including lactose intolerance and irritable bowel syndrome.

There are different yogurts to choose from, such as Greek yogurt which is thicker than regular yogurt.

Add a zero-calorie sweetener (or a teaspoon of honey), nuts or sunflower kernels or a sprinkle of whatever spices that sound good — cinnamon, cardamom, or ginger. One serving of two per cent Greek yogurt is 170g.

3 - Beans

YOU can’t go wrong with beans. Beans are high in fibre and protein and are a good source of vitamins and minerals, such as folate, iron, magnesium and potassium, which is essential for the water balance between the cells and body fluids.

There are many delicious varieties to choose from, such as black, kidney, garbanzo, white, lima, and pinto. Soak and cook dry beans or use canned beans. Try serving beans as your main protein source for lunch or dinner a couple times a week. Protein is an important part of your daily nutrition and it helps the body repair and produce cells as well as build muscle and bones.

The American Diabetes Association counts one serving of beans, or ½ cup, as one starch and one lean meat.

4 - Broccoli

DON’T underestimate the nutritional power of broccoli. Truly a super food, this non-starchy vegetable has more vitamin C per 100g than an orange and is considered a good source of fibre and the antioxidant beta-carotene, which the body uses to make vitamin A. Its vitamin A power promotes healthy vision, teeth, bones and skin. Vitamin C is essential for healing wounds and is a disease-fighting antioxidant.

One serving of broccoli is one cup raw or ½ cup cooked.

5 - Carrots

COOKED or raw, carrots are a healthy addition to any meal plan. Have them for a snack with two tablespoons of light ranch dressing or include them in your main course or as a side dish. It provides vitamin A from the antioxidant beta-carotene. Carotenoids found in yellow and orange vegetables may also help reduce insulin resistance. Carrots are also another source of fibre and heart-healthy flavonoids, which can also be enjoyed as a juice with other fruits and vegetables such as apples, beets, and ginger.

One serving of carrots is one cup raw or ½ cup cooked.

6 - Fish

FISH is a great addition to your meal plan, especially Omega 3-rich fatty fish such as salmon, trout, tuna, sardines, mackerel and herring. Omega 3, a type of polyunsaturated fat, can help lower triglycerides. It is said to be able to reduce inflammation, lower blood pressure and reduce the risk of blood clots. Although fish is good for you and is considered a lean-meat substitute for its high protein, be careful of the harmful mercury levels and other toxins found in some fish. According to the American Heart Association, swordfish, shark, golden bass, golden snapper, and king mackerel have the highest mercury levels. Fish lower in mercury for a 85g serving include wild salmon, herring, catfish, and canned light tuna. Try preparing fish on the grill, baked, broiled, or steamed. One serving of fish is 28g.

7 - Oatmeal

A BOWL of oatmeal in the morning will keep you full for longer, getting you to your mid-morning snack or lunch. The soluble fibre in oats can help lower cholesterol, improve blood pressure and stabilise blood glucose by slowing digestion. Oats are also a source of antioxidants.

Flavour oatmeal with cinnamon or artificial sweeteners to keep calories low. Oats also provide vitamin E and B as well as magnesium and potassium, which may help lower blood pressure.

There are several types of oatmeal to choose from. Steel-cut oatmeal has a dense, thick texture and can take up to 45 minutes to cook, while old-fashioned (or rolled) oats are thinner and take less time to cook. The less processed the oat, such as steel-cut oatmeal, the lower it is on the glycemic index, which may help control blood glucose. Quick cooking oatmeal and instant oatmeal are also available. Be sure to check the labels for added salt and sugar. One serving of oatmeal is ½ cup.

8 - Spinach

POPEYE ate spinach for a reason. This dark green leafy vegetable is loaded with vitamins and minerals, including vitamins B2 and B6, folate, copper, magnesium, potassium, zinc and fibre.

Studies have found that spinach has the potential to decrease the risk of cancer, cataracts and heart disease. Spinach is high in beta-carotene, an antioxidant that the body uses to make vitamin A. Beta-carotene helps protect the body’s cells from free radicals, which contribute to chronic illness and ageing.

When buying canned spinach, choose a low sodium one. One serving of spinach is ½ cup cooked or one cup raw, which is great for salads.

9 - Soya

OFTEN used as a substitute for animal products, soya is an excellent power food to incorporate in your diet, even if you aren’t a vegetarian. Soya can be eaten in whole bean form, such as baby green soya beans called edamame, which is the highest in protein. Other soya products include soya milk or cheese, tofu, soya nuts, or vegetarian meatless products. Soya is also a source of niacin, folate, zinc, potassium, iron, and alpha-linolenic acid (ALA), a fatty acid that can be converted into Omega 3 fatty acids, known to help lower cholesterol. It also contains nutrients such as niacin, zinc, potassium and folate that serve important functions in the body. However, do check with a health-care professional before increasing potassium intake if you have kidney complications or kidney disease.

Serving sizes depend on whether soya is consumed in food or drink.

10 - Nuts

NUTS are a good source of protein, fibre, vitamin E, and flavonoids and are power-packed with monounsaturated fat.

Plant sterols known to lower cholesterol also naturally occur in nuts. But be careful. Although nuts contain healthy fats, they are also high in calories. Walnuts, almonds, macadamia nuts, pecans and hazelnuts are just some of the nuts that can help lower LDL (bad) cholesterol, making them heart-healthy choices. Eat nuts in moderation and avoid salted, sugared or chocolate-covered options that increase calories and decrease their natural health benefits. One serving of almonds, cashews, or mixed nuts is six nuts. One serving of pecans is four halves, a serving of hazelnuts is five nuts, and a serving of pistachios is 16 nuts, as per the American Diabetes Association.

Quit Tips

Are you one of most smokers who want to quit? Then try following this advice.

1. Don’t smoke any number or any kind of cigarette. Smoking even a few cigarettes a day can hurt your health. If you try to smoke fewer cigarettes, but do not stop completely, soon you’ll be smoking the same amount again.
Click to read page in Spanish

Smoking "low-tar, low-nicotine" cigarettes usually does little good, either. Because nicotine is so addictive, if you switch to lower-nicotine brands you’ll likely just puff harder, longer, and more often on each cigarette. The only safe choice is to quit completely.

2. Write down why you want to quit. Do you want to—

* Feel in control of you life?
* Have better health?
* Set a good example for your children?
* Protect your family from breathing other people’s smoke?

Really wanting to quit smoking is very important to how much success you will have in quitting. Smokers who live after a heart attack are the most likely to quit for good—they're very motivated. Find a reason for quitting before you have no choice.

3. Know that it will take effort to quit smoking. Nicotine is habit forming. Half of the battle in quitting is knowing you need to quit. This knowledge will help you be more able to deal with the symptoms of withdrawal that can occur, such as bad moods and really wanting to smoke. There are many ways smokers quit, including using nicotine replacement products (gum and patches), but there is no easy way. Nearly all smokers have some feelings of nicotine withdrawal when they try to quit. Give yourself a month to get over these feelings. Take quitting one day at a time, even one minute at a time—whatever you need to succeed.

4. Half of all adult smokers have quit, so you can— too. That’s the good news. There are millions of people alive today who have learned to face life without a cigarette. For staying healthy, quitting smoking is the best step you can take.

5. Get help if you need it. Many groups offer written materials, programs, and advice to help smokers quit for good. Your doctor or dentist is also a good source of help and support. See a list of National Groups with information and resources on how to quit.
